What is the Supplementary Information Form?

The Supplementary Information Form is an extra form that some parents need to fill out when applying for a place at St. George’s Church of England Primary School and wish their application to be considered under the following admission criteria:

  • Criteria 3: If you and your child regularly attend St. George’s Church or another Christian church and live within the specified area.
  • Criteria 5: If you and your child regularly attend a Christian church but live outside the specified area and do not have a more local Church of England school.

What information is needed on the Supplementary Information Form?

The Supplementary Information Form asks for additional details to confirm regular attendance at church. This includes confirmation from the clergy or church leader that you attend church at least once a month for a year.

When and where to submit the Supplementary Information Form?

The Supplementary Information Form must be returned to the school office of St. George’s Church of England Primary School by the deadline for the local authority application process.

Your general application for a school place must be submitted online using the Apply for a school place form or the Birmingham School Admissions for Reception 2026 places.
